It's always puzzled me that people are so readily willing to accept medical advice from politicians and government employees. When did we start trusting these people? I wouldn't buy a used car from Joe Biden or Tony Fauci, never mind accept medical advice from them. Thoroughly dishonest, corrupt, and untrustworthy people the lot of them.

I haven't had any kind of vaccination / innoculation / jab in 40 years. The last vaccination I had was a tetanus shot when I was a teenager after I punctured my foot on an old nail. No covid, no flu, no shingles, no thanks. I'm doing just fine.

I was a kid in the 70s when the swine flu was going around and more people died from the flu shot than from the flu. That made quite an impression on my young mind. I always err on the side of less medicine.

I'm not quite as jaded as you. But I'm on that road. I'm currently weaning myself off a statin. Next will probably be the baby aspirin and the blood pressure med. All of these prescribed for "priamry prevention" of circulatory disease. Sounded good when Doc wrote the script. But I've been doing some research. Turns out that those interventions have very little expected benefit for the average patient and are quite possibly harmful. Come to think of it, the mRNA jab is a perfect example of this phenomenon. I'm doing this behind their backs. At some point I will tell them that I no longer trust their profession. I'll even offer to show them the data. But since they are some combination of brainwashed and/or legally required to prescribe me potentially harmful treatments that I probably don't need, I doubt they'll accept my offer. The last thing to negotiate is whether I should continue being their patient or perhaps I will be fired. Either is fine with me. I'm still willing to undergo routine testing and such, but I will view with a very skeptical eye any drug prescribed me. I suppose I'd make an exception in a serious emergency, but for routine care? Let the buyer beware.

This is a pretty good example of a little bit of information, misapplied, being dangerous.

The Statin family have proven to be effective in setting a target number. What the number means is largely misrepresented or misunderstood. The bulk of studies show lower the number can slow progression of arterial disease. This is extrapolated to presume that lowering the number in general, in the absence of disease, will reduce risk. We all know extrapolation in this context is not mathematically valid. The problem is that no one's figured out how to tell if a person's physiology is prone to developing arterial blockage. So the logic is that IF you might be one of those people prone to develop blockages, reducing cholesterol in your blood will prevent disease from developing - not well supported by real study but a common argument of physicians because it is the guidelines they've been given - hit the target number by whatever means available.

I have this ongoing debate with my doc over this. My number comes in just over the recommended max. The first round, I asked for more justification than "because we can hit the number with statins". So he ordered a cardio workup. The cardiologist shook his head in near amazement (or was it disappointment?) as he noted my CV function scores what would be good for a 30 year old. I'm over 60. And my exercise level has been hampered since 2020 due to COVID closures and some dumb injuries. I expect we'll continue the debate..

Now for the even darker side of statins. While one might believe lifestyle and diet in particular are them main affecters in cholesterol, this isn't the case. The liver plays a dominant role. There is considerable evidence that liver problems can cause high cholesterol. But most MDs will simply "treat" the high cholesterol with statins, instead of look for an actual cause. This is particularly worrisome when there has been a change in cholesterol, which may be an early indication of liver problems. So the use of statins masks the indication. That's never a good thing.

I’ve got bad news for you. Nothing you do (medically speaking) is done behind your doctor’s back. The insurance companies track your use of Rx meds and if you don’t refill your meds on time, they send an alert to your PCP or whoever wrote the prescription. I know, because I get these reports on a daily basis. Of course then I’m expected to “reach out” to the patient and determine why they are being non-compliant. Is it an issue of affordability? Maybe they are experiencing adverse effects? Or maybe, like you, they don’t see the need for these meds. And like many practitioners, I pitch these alerts in the trash. I don’t have the time or the inclination to tell people how to live their lives, and I’m not about to force people to take medications that they don’t want. I just wish they would tell me up front before a prescription is written, so we could talk about alternative treatments (diet, exercise and weight loss would fix about 90% of the health problems that most adults suffer from). But most folks don’t want to hear about that, they want a quick fix, and “that pill that my cousin takes” that helps their (fill in the blank).

I hear the "diet, exercise and weight loss" litany frequently, but the more I learn, the more I realize that the usual advice is not based on sound science. In deed, vast amounts of data show that boilerplate dietary advice in general does not work for a lot of people. Many medical practitioners simply reject anything that deviates from the standard advise. You seem like a more thoughtful practitioner, so I'd be interested in your view. I don't do things behind his back, we talk it out and if I do not agree I say so. I may be one of those "non-compliant" patients of which you speak (probably a good label overall ;-0).

Part of the issue may be defining "health problems". The industry seems to fixate on what metrics that are frequently misapplied or simply invalid (such as BMI). The main issue I have with typical medical practice is lack of seeking root causes. I understand that human physiology is complex and not well understood, and the statistical variation in physiology is vast. So there are times "try this and see what happens" is as good as it gets. But this seems to lead to a habit of treat with the usual drug rather than understand underlying causes. Sometimes there really more to the story than "get the number we want".

As a thinking medical doctor, how do you deal with the potential conflict between the "do no harm" principle, the use of drugs who's mechanisms are not fully understood, pressure to follow government and AMA "recommendations" and the desire to give quality care to the patient?

Thank you for the response. I try to simplify my life (and my patient’s) as much as possible. I start by ignoring almost all of whatever tripe the AMA and various specialty boards are pushing, since every one of these groups are beholding to Big Pharma. I despise “algorithms” and tend to ignore “evidence-based” recommendations unless they make sense for a particular patient. (TPTB are doing their best to turn medicine into some cookie cutter process that could be managed by a group of fairly well trained chimps.) Goals are nice, but not absolute. My quarterly “report cards” from the “healthcare system” I am employed by are fairly reflective of this as I rarely exceed the 30-40% mark in any of their metrics.

I’m also somewhat of a libertarian, I don’t do “paternalistic” medicine well, so I don’t argue with my patients. “Screening” for various preventable illnesses is something I’m in favor of, but in some people it’s extremely anxiety provoking and if they refuse, I don’t make an issue out of it. I do have discussions about their particular ailment, if it can be corrected, and what can be expected in the long term if it isn’t. When it comes to the biggies (hypertension, high cholesterol, diabetes and obesity), I am not a stickler for meeting arbitrary goals but rather for at least improving the numbers. The only time I really insist on using medications are when the numbers are alarmingly high.

When it comes to bad habits (smoking/drinking/drugs) while recovering completely is always the goal, if I can nag, wheedle and guilt people into at least cutting back, I feel like it’s a small victory. Baby steps. Other ailments are generally associate with physical discomfort of some kind and most patients need/want/expect some type of medication (either short or long term).

I try my best to avoid harming patients (either physically or emotionally) and if they feel they are being injured in some way by anything I’ve prescribed, they know that they can discontinue that treatment although I ask them to notify me if that is the case. But not all patients will do that, because (I suppose) they don’t want me to feel like I failed at “fixing” them.

Despite all of this, most of my patients tend to live long enough to meet or exceed the average life expectancy. I’m not sure if that is something I can take any credit for but I’d like to think so.

My PhD Thesis was on advanced medical imaging (CT and MRI) and I like empirical results (measuring things ;-). So I tend to like screening, testing and imaging. Data is good.

I'm also not against medications when needed. My analytical brain likes explanations that make sense.

I have a friend and former colleague who is a geneticist. Regarding long life, his advise is exercise, eat right, and choose your parents wisely. The first two don't affect quantity, only quality. The genetics is what matters most - not a surprise coming from a geneticist. FWIW.

Also the same folks who call for MORE regulation by the government. Same folks who in 2020 decried the "fast track" program of the Trump administration as circumventing regulations (it did) and swore they'd never take the "Trump vaccine" and suddenly in 2021 proclaim we MUST all take the vaccine and we all trust the pharma companies?

Almost as entertaining as listening to the same folks explain why uncensored free speech is anti-democracy.

Always test the edge before buying.

Speak truth, if you have to speak, but always listen for the lie.

But then, one can't go far wrong if heeding the wisdom of Allfather Oden, Frigg the Wise and the other gods.

Tell the truth? My father, a scrupulously honest man and a career bureaucrat, if those two can be reconciled (it was a LONG time ago!!!) said "Always tell the truth, but not more of it than is required."
