Ellis island was a quarantine facility. All arriving 3rd class passengers were automatically quarantined on the island. Those who displayed symptoms of disease were sent back to their home countries.
While I love the movie “Titanic” as it showcased the ship before she sank, there was no way the two main characters could have met. By US immigration law, ships were constructed in such a way to physically isolate 3rd class passengers AND the crew that served them from the rest of the ship. Once one boarded the 3rd class section there was no access to any other part of the ship for passengers nor crew. When ships arrived in the US, 3rd class passengers went into quarantine. Quarantines are the restriction of the healthy. Isolation is the restriction and treatment of the sick.
However, this disease does not call for a quarantine. I am not anti vax but am being pushed in that direction. The medical community has done much to earn the scorn of the general public. Never one to just blindly trust doctors, at this point though it is hard to trust them on anything.
There was a possibility to control covid by completely shutting borders except for returning citizens and enforcing an appropriate quarantine on those who returned. But that chance was denied by China first trying to keep it secret, then lying and saying it was not transmitted person to person and allowing international travel while prohibiting domestic travel. Once the disease got in, you are right, there was not and is not any way to control it.
I am not sure from reading your comment if you are refering to the historial events but the yellow q flag doesn't mean disease anymore. It just means that you have arrived in a new country and haven't gone through customs yet.

Yes, I was referring to historical instances before radio when signal flags and lanterns where the only way to communicate with the shore. Well, I guess a hailer too but you’d have to be a lot closer than quarantine would allow.

BTW, is a quarantine flag even used these days. I have some knowledge of how things were in the sail era but next to none now. Well, outside the engine room that is.

There are international signal flags (https://www.boatsafe.com/international-code-signaling-flags/) but I have only see A, B and Q in use. I would use radio communications for most of the situations instead.
