(Actually, my read of the trial data does indicate that all-cause mortality was worth taking a closer look at, but...)
As to how the orthodoxy survives, history is chock-full of practices and custom that seem obviously flawed in hindsight that were almost worshiped at the time. Unfortunately, we are living through an example.
